Wildcat Student Health Clinic
HOW DOES IT WORK?
- Parents opt-in by returning the signed consent form to school & calling the Union County Health Department to book the appointment: 937.645.2058.
- In Mid-September, the health department will call parents to confirm participation & verify the services their child will receive.
- children with signed & Verified consents are seen by a licensed healthcare provider at the one-day school health clinic. A follow up report is sent to the parent.
